Patients wishing to register with First Medical must complete a registration form. This can be done here on our website. Alternatively, you can download the form in word format, print it out and bring or email it to the surgery.

For the registration you will need the following documentation: photographic ID, recent utility bill and Social Security Green Health Card (without this card you will be classed as a Private Patient).

Once your registration is processed you will be notified by the team.

Before booking an appointment you MUST book a patient health screen with our HCA.

Please note that, for the first month, you will be classed as ‘Pre-registered’ and we will require payment at the time of requesting a service.

Should you wish to request medication you MUST have had your first GP face to face appointment

All new patients should bring a list of their repeat medications to their first appointment.

When completing this form make sure you have with you your photo ID, proof of address within last 3 months and green health card. For change of name legal documents must also be provided
